Just throwing out an idea: What about noise mufflers, such as those used at firing ranges or by airport workers, worn, like headphones, over the "offending" ear? One would think that should cancel both the outgoing noise of the hearing aid, as well as the incoming noise from the external sound system, and, hopefully, cancel the positive feedback loop of increasingly self-amplified sound that may be developing (aka "feedback"). At ca. -30dB (in and out) passive sound insulation, would this or a similar approach would do the trick? If the concept works, then, one can move to a more customized, "stylish" and yet functional design.
