We went through this process a few years ago.  We originally started with a very lax policy and cell phones and services were paid for a university owned devices.  That has changed greatly and is covered pretty comprehensively in the links below.

We had a cell phone policy and even gave stipends but now this is a condition of employment (IT staff members must have a cell phone and be accessible for after hours support).

We've just completed the transition to stipends for those staff who require mobile devices to perform their responsibilities.  The policy is titled, "19.6.B Policy on Cellular Telephone Service" and is available here: http://www.miami.muohio.edu/documents/secretary/MUPIM.pdf  (Scroll down the table of contents to section 19.6, the very last item in the list, and click).
